Covington Community School Corporation has partnered with Saint Mary-of-the-Woods College family nurse practitioner program and will once again be offering FREE athletic physicals Monday through Friday, June 16 – 20. Times slots will run from 9:00 A.M. – 3:50 P.M. with 30 minutes allotted for each physical.
FREE Athletic Physical Appointment Registration Process and Appointment Notes
1. Copy and paste or click on the link in the body of the email into your browser to schedule your appointment: https://forms.gle/tvRE7nwkpj2p9SRZ7 (appointment times are confidential and cannot be seen by others who are registering for an appointment)
2. On the day of your physical, please arrive 15 minutes prior to your official time slot to complete pre-physical paperwork.
3. 2025-26 IHSAA Physical Forms will be available at CHS on the day of your physical or you can print one from our website (www.covington.k12.in.us).
4. A parent/guardian must accompany each student-athlete to their appointment to sign paperwork.
5. Student-athletes should report to Door #1 (HS main entrance) for their appointment.
Student Spectator Expectations:
1.) Once you enter the building / complex, you are not permitted to leave and come back (unless you talk to administration).
2.) No backpacks / outside drinks.
3.) You must stay in the gym or approved areas. No running around in the hallways, or behind the bleachers.
4.) Students in sixth grade or below, must be accompanied and supervised by an adult during the entire game.
5.) Students must show respect at all times.
If you break these expectations, you will be asked to call home and leave. School consequences will also be enforced as necessary.
