Before an athlete is allowed to participate at any level of practice or competition he/she must have:
(1.) Complete physical (form available below).
(2.) Record of insurance coverage on file in the Athletic Office.
(3.) Emergency Medical Authorization Permit on file in the Nurse's Office (form available below).
The school reserves the right to require an athlete to undergo a follow-up physical if the initial exam indicates any potential physical problems. The follow-up exam will be at the schools expense. |
