To: CHS Athletes and Families From: CHS Athletic Department Re: 2020-21 Athletic Physicals Requirement
As we move toward opening our IHSAA athletic seasons for the 2020-21 school year, there are a number of topics that will need to be addressed with the safety of our student-athletes in mind. It has been announced that the IHSAA is allowing schools to utilize their 2019-20 athletic physicals for 2020-21, but with safety in mind, it is the position of the CCSC administrative team that the Covington High School athletic department will require a new physical from our athletes for the 2020-21 athletic year.
To assist our high school athletes with a pathway to obtain a new athletic physical, we have partnered with Saint Mary-ofthe-Woods College family nurse practitioner program to offer FREE “Athletic Physical Days” at Covington High School on June 24, 25 (half day), 26, 30, July 1, and July 2 (half day), 2020.
Athletic physical time slots will run from 8:00am – 5:30pm on the above full days and 8am-12pm on the two half days with 45 minutes allotted for each physical. Please see the appointment registration process below. If a student-athlete would prefer to secure an athletic physical from their own family doctor/medical provider, that is acceptable.

On the day of your physical, please arrive 30 minutes prior to your official time slot to complete pre-physical paperwork. (15 minutes prior if you already have a physical form filled out from home)
2020-21 IHSAA Physical Forms will be available at CHS on the day of your physical.
A parent/guardian must accompany each student-athlete to their appointment to sign paperwork.
Student-athletes should report to Door #13 (south side of building) for their appointment.
Student-athletes and their parent/guardian are asked to bring a face mask to comply with CDC guidelines in making sure everyone at the school operates in a safe manner.
Athletic physical timeline – As has been the case in prior years, an athletic physical must be on file in the CCSC athletic department prior to the first IHSAA practice for your fall, winter, or spring season (whichever is your first season).
CHS student-athletes only – Due to the limited number of time slots available, this round of athletic physicals is reserved for incoming 9th – 12th grade student-athletes.
Thank you for your efforts on this matter as we work collaboratively to keep our kids safe and productive in their athletic endeavors.
